As you may know, the Authority is ramping up efforts to begin COVID-19 vaccinations of our workforce through the MBTA’s Occupational Health Services. Filling out this form will assist the T in knowing how many vaccines are necessary and will allow for the Authority to plan a distribution schedule for those who want it.
